Medical certificate from Govt. Hospital/Dispensary (As per the AIEEE format) Affidavit - Citizen of India (as per AIEEE format), If already submitted to PTU, then photocopy of the affidavit. Four latest Passport Size Coloured Photographs. Migration Certificate in Original other than Punjab School Education Board.
